A higher quality of life and improved health and wellbeing by working differently with health and social care institutions to create a locally owned and accountable community health service – a new model for primary care and GP surgeries.

• Create an innovative community owned primary care service with a GP surgery at its heart offering appropriate provision that reinvests surpluses locally to offer increased wellbeing. (NHS England and other health partners)
• Develop a community owned social care provider / care home and community-based service to promote independence/person-centred care and pay staff a competitive wage. All surpluses to be reinvested to offer increased wellbeing opportunities for local people.
• Implement new approaches to physical and mental wellbeing by supporting our flourishing social action and community businesses, forming a vital plank in our community owned primary care service, with properly resourced social prescribing.
• Work with local partners to create a Hartlepool Half Marathon as a mechanism to inspire being more active.
